For this application to proceed to the Energy Efficiency Staff for review, please complete this incentive application and upload the required documentation below:

Photo of the installed EV Charger
Photo of Charging Schedule
Equipment or Contractor Invoice showing:
- Invoice Date
- Installation Cost
- Model No.
- Charger Type
- Quantity of Chargers being installed
- L&I Permit and passed inspection report

Once the application is reviewed, an email notification stating either “Approval” or “Declined” will be sent to the Customer’s Email on the application.

If approved, please allow up to six (6) weeks for the incentive to process. Thank you.
